Fallopian tube pathology

Congenital anomalies of the fallopian tubes are possible, but more often they occur after infectious and inflammatory diseases. Obstruction of the fallopian tubes or narrowing of the lumen develops.

Determination of patency is carried out using fluoroscopic examination (with the introduction of a contrast agent) - hysterosalpingography, which makes it possible to determine the shape of the uterine cavity and the patency of the tubes. Examinations are carried out using a hysteroscope - an optical system that is inserted through the cervix into the uterus.

Treatment. Drug treatment is carried out (antibiotics, enzymes - lidase), usually in combination with laparoscopy - excision of adhesions. What is laparoscopy: a contrast solution is pumped into the uterus and tubes under pressure (it cuts the adhesions), and an X-ray of the pelvis is taken to check the result. It hurts a little, but tolerable. To stop bleeding after the procedure (due to damage to the internal lining of the uterus) and to prevent infection of the damaged uterus, tampons with an antibiotic (usually Trichopolum) are placed for 7-10 days.
If the obstruction is caused by trauma, surgery is performed, but this method is used infrequently and is ineffective.

Pathology of the cervix

The cervix secretes mucus, which is a biological filter. It prevents microflora from the vagina from entering the uterus, and during ovulation it increases the viability of sperm. The consistency, composition and viscosity of cervical mucus change under the influence of estrogens in accordance with the phases of the menstrual cycle. In the first half of the cycle, little mucus is formed; in the middle of the cycle it becomes more viscous and abundant. Then the mucus becomes watery and its quantity decreases, causing vaginal dryness.

Possible violations:
Excessive thickness and viscosity of mucus, which interferes with the movement of sperm.
The presence of antibodies to sperm in the mucus, causing their agglutination (protein folding) and inability to fertilize. The methods allow one to evaluate the composition of mucus and the survival rate of sperm in it. A compatibility test is carried out in the middle of the cycle, when the conditions for fertilization are most favorable. A vaginal mucus sample is taken from 2 to 8 hours after sexual intercourse.

Treatment: selected individually. May include drugs to reduce the viscosity of mucus, or intrauterine insemination (fertilization by introducing seminal fluid directly into the uterine cavity).

Folk remedies for treating infertility:

  • Pour 1 tablespoon of adonis herb with 1 glass of boiling water, leave, wrapped, for 2 hours and strain. Drink 3 times a day as tea for infertility.

How to cure infertility

November 2005. After treatment, dysfunctional bleeding did not stop. The doctor ordered a blood test for hormones.

Thus, hyperprolactinemia was discovered (prolactin (PRL) is much higher than normal) and minor abnormalities in the thyroid gland (st T4, which is why I lost weight). The endocrinologist prescribed Bromocriptine with monthly monitoring of PRL in the blood with dose adjustment and L-thyroxine, also under the control of a blood test, an individual dose is selected.

I took an X-ray of the brain and the fundus was examined by an ophthalmologist, everything was fine.

I measured my basal temperature every month.

After 2 months, the hormones returned to normal while taking the drugs (321), but it was necessary to take the pills under the supervision of a doctor for a long time.

After six months, bromocriptine was gradually reduced in dose and was discontinued, and after discontinuation, my BPD jumped sharply to 2000, bromocriptine was prescribed again.

At the same time, a general blood test and serological analysis were always normal.

They took an x-ray of the uterus, tubes and ovaries with a contrast solution - the tubes were contrasted, everything was normal.

In the first half of the cycle, we observed by ultrasound how the follicle was maturing - the size of the follicle reaches 20-21 mm, i.e. there should be ovulation. Judging by the basal temperature graph, there is insufficiency of the second phase of the menstrual cycle. I didn’t take any additional tests to confirm this, but I was prescribed Duphaston to drink for 10 days from the start of the upward jump in the temperature curve. Dysfunctional bleeding stopped immediately!

I forgot to say about my husband’s sperm: a sperm analysis showed that there were 50 to 50 mobile and immobile sperm in the sperm, so I was prescribed to drink “Speman” (some kind of herbal tablet). By the way, after taking them, desire increases greatly, so get ready to have sex often.

Looking back and summing up the results at the moment, we have: a year has passed since the first visit to the doctors. In parallel with the main treatment, I began to try folk remedies for infertility:
To begin with, I cleaned all organs and systems.
I drank tea from sage in the first half of the menstrual cycle, tea from the herb boron uterus. - no changes.

According to ovulation tests, ovulation occurs, but there is no pregnancy.

I started drinking celandine tincture and it just went away.

The doctor recommended a position for conception, taking into account the fact that the uterus is tilted backwards - on all fours, the man behind, and stand like that for a while, then lie for half an hour on the side where the follicle was mature (according to ultrasound). July 2007. Stimulated until a cyst appeared in the right ovary (consequences of ovarian hyperstimulation). The dose of duphaston was increased. The cyst was gone by the next cycle.

August 2007. I came across an article that infertility is a consequence of bad karma and we need to look for the cause in past generations, I rummaged through my memory.   4. As a result of increased blood clotting, thrombosis of the pelvic vessels, thromboembolism of large vessels,
  5. Multiple pregnancy.

There are 3 forms of severity of ovarian hyperstimulation syndrome (OHSS). In mild forms, patients feel well, the diameter of the ovaries does not exceed 5 cm. In moderate forms of the syndrome, women report pain in the lower abdomen, heaviness, nausea, vomiting, and diarrhea. The diameter of the ovaries, according to ultrasound, is from 5 to 12 cm. The severe form of OHSS is accompanied by a pronounced accumulation of fluid in the abdominal and pleural cavities, where the lungs are located, increased blood clotting, decreased blood pressure, increased heart rate, impaired renal function up to renal failure .

When pregnancy occurs, OHSS is more severe than in its absence. In moderate and severe forms of OHSS, a woman needs hospitalization and constant medical supervision. In the hospital, intravenous infusions are carried out aimed at restoring the volume of circulating blood, stimulating kidney function, drugs are prescribed that reduce vascular permeability, and if blood clotting increases, drugs that normalize it are prescribed. The frequency of recurrent ectopic pregnancy after microsurgical operations on the fallopian tubes reaches 25%. If the pregnancy test is positive, it is necessary to confirm the presence of a fertilized egg in the uterine cavity using ultrasound. If a woman had no other causes of infertility and the fertilized egg ended up in the uterine cavity, then such women have no more complications during pregnancy than other pregnant women.

Consequences of various gynecological diseases

If a woman had sexually transmitted infections before pregnancy, then even if they were not detected before pregnancy after therapy, they can worsen during pregnancy, which is associated with a weakened immune system that is common in pregnant women.

Therefore, after pregnancy, tests are taken again for those infections that the patient suffered (smear, culture of secretions from the genital tract, blood for a viral infection), ultrasound monitoring of the state of the amniotic fluid, the fetal placenta is carried out (indirect signs of intrauterine infection may be the presence of a suspension in the amniotic fluid waters, polyhydramnios, “thick” placenta, accumulation of gases in the fetal intestines, etc.). If an infection is detected, antibacterial therapy is carried out (from 14-16 weeks of pregnancy), in case of a viral infection, immunoglobulins are administered intravenously, ozone therapy, immunotherapy with drugs approved during pregnancy and therapy aimed at improving the general condition of the mother and baby are carried out.

Infertility with uterine fibroids(benign tumor of the uterus) is most often the result of inflammatory diseases of the uterus and appendages (which can result in obstruction of the fallopian tubes, chronic endometritis - inflammation of the uterine mucosa) or changes in the function of the endometrium (inner lining of the uterus) as a result of hormonal disorders. When pregnancy occurs in the first trimester, rapid growth of fibroids is observed, but it often stops at 16 weeks. This is due to hormonal changes in the body that occur during pregnancy. Sometimes swelling and necrosis (death) of the node occur. Constant nagging pain appears in the lower abdomen.

If for swelling of the myomatous node conservative treatment is carried out, then for necrosis - careful removal of the tumor against the background of drugs that relax the uterus and antibacterial therapy. During pregnancy, there is often a threat of miscarriage, mainly during the period of implantation (when the fertilized egg penetrates the mucous membrane and attaches to the wall of the uterus), then at 16-18 and 36 weeks of pregnancy. With uterine fibroids, insufficiency of the 2nd phase of the menstrual cycle is very often determined, therefore, if the level of progesterone in the blood is low, progesterone analogues are prescribed until 14-16 weeks of pregnancy.

With uterine fibroids, hypoxia and fetal growth retardation are possible, mainly in the case of attachment of the placenta to the myomatous node, especially if the node tends to grow towards the uterine cavity.

In cases of pregnancy occurring after removal of a myomatous node, if the placenta is located along the scar (especially if the operation was performed with the opening of the uterine cavity, in the postoperative period there were inflammatory processes in the uterine cavity, curettage), fetal hypoxia, fetal growth retardation due to placental insufficiency are also possible , abnormalities of placenta attachment, threat of miscarriage (in 1/3 of women), incorrect position of the fetus in the uterine cavity. During pregnancy, ultrasound monitoring of the condition of the scar, fetal size, and cardiotocography (assessment of fetal cardiac activity) is performed.

Abnormal placentation (incorrect attachment of the placenta) often occurs not only with uterine fibroids, but also with chronic salpingoophoritis- uterus, endometritis- inflammation of the mucous membrane of the uterus, cervicitis - inflammation of the mucous membrane of the cervical canal, cervical erosion, endocrine diseases, in cases where a woman has suffered curettage of the uterus etc. In these cases, either placenta previa occurs (the placenta covers the internal os of the cervical canal), or its low attachment. If abnormal placentation is detected, the woman undergoes an ultrasound once every 4 weeks. Placenta migration occurs very often (as the uterus grows, the placenta moves upward). This process may be accompanied by the threat of miscarriage, bleeding from the genital tract, and anemia. Such phenomena require treatment and observation in a hospital. (FPN) - a condition of the placenta and fetal-placental circulation, in which the baby does not receive enough oxygen and nutrients - occurs in 2/3 of women with abnormal placentation. In 1/5 of women, fetal hypotrophy develops due to FPN, so constant monitoring of fetal size and assessment of fetal cardiac activity is required for timely detection of signs of hypoxia.

Consequences of impaired spermatogenesis

If fertilization of the egg occurred initially with an inactive, defective sperm, which brought incorrect genetic information into the egg, leading to the formation of gross chromosomal pathology in the embryo, there is a possibility of a non-developing pregnancy in the early stages (mainly up to 6-8 weeks).

A woman must undergo a timely ultrasound, during which the size of the ovum, the presence of a fetal heartbeat, etc. are assessed.

Pregnancy after immune infertility

In this situation, pregnancy usually occurs after artificial insemination with the husband's sperm, when, after special treatment, sperm are introduced into the uterine cavity, bypassing the mucus barrier in the cervix, where the concentration of antibodies that glue them together is especially high. In this case, pregnancy usually proceeds without complications.
