2) from medical organizations providing primary health care - by referring patients to medical commissions for hospitalization in rehabilitation hospitals.

THIRD STAGE OF MEDICAL REHABILITATION

Referral of patients for medical rehabilitation in an outpatient setting (polyclinic, day hospital, sanatorium) to stage III is carried out:

• from traumatological, orthopedic, rheumatological and neurosurgical after the end of hospital treatment;
• from inpatient medical rehabilitation centers/departments;
• from medical organizations providing primary health care.

From medical organizations providing primary health care that are not included in this list, patients should be sent to branches No. 1, 2 and 7 of the State Autonomous Institution of Public Institutions Scientific and Practical Center MRVSM DZM.

Patients referred for medical rehabilitation must have with them:

• passport of a citizen of the Russian Federation or identity document;
• compulsory medical insurance policy;
• direction (057/у-04);
• a copy of an extract from the medical record of an inpatient or outpatient patient;
• radiographs (not more than 6 months old), CT or MPT;
• ECG results, up to 1 month old;
• clinical blood test (up to 1 month old);
• general urine test (up to 1 month old);
• results of fluorography or chest x-ray (up to 1 year old);
• a gynecologist's report (for women) up to 1 month old;
• blood test for hepatitis B and C (up to 1 month old);
• blood tests for HIV, RW (up to 1 month old).

Sanatoriums

Follow-up treatment (rehabilitation) in sanatoriums is provided to patients after orthopedic, traumatological operations for defects and malformations of the spine, intra-articular fractures and joint plastic surgery, endoprosthetics and re-endoprosthetics, replantation of limbs in satisfactory condition, in the absence of postoperative complications, not requiring dressings, capable of independent movement , self-care, with a level of physical, mental and mental abilities sufficient to stay in a sanatorium without constant outside care.

The medical selection of patients sent for follow-up treatment is carried out by the medical commission of the hospital where the first stage of rehabilitation was carried out.

As of 2017, follow-up treatment of patients after injuries and operations is not carried out by:

• Sanatorium of the Office of the Mayor and Government of Moscow named after Artyom (Sergeev).
